Healthcare Finance Calculators

Hospitals and health systems are measured with a specialized set of metrics: liquidity in days cash on hand, revenue-cycle speed in days in accounts receivable, volume in adjusted patient days, acuity in case mix index, and debt capacity through covenants. Free tools for these measures are rare.

These calculators apply the definitions used by hospital finance teams, lenders and rating agencies, and each page notes where covenant or regulatory definitions may differ.

The 9 calculators

Days Cash on Hand

Measure how many days a hospital or health system could cover cash operating expenses with unrestricted cash, and test the result against a debt covenant.

Days in A/R & Net Collection Rate

Measure revenue-cycle performance: days in accounts receivable, net collection rate, gross collection rate and the cash tied up versus a target.

Cost per Adjusted Patient Day & Adjusted Discharge

Convert inpatient volume into adjusted patient days and adjusted discharges that reflect outpatient activity, then measure cost, revenue and margin per unit of service.

Case Mix Index (CMI) Calculator

Calculate the case mix index from MS-DRG relative weights and discharges, see which DRGs drive acuity, and estimate DRG payments and CMI-adjusted cost per discharge.

FTEs per Adjusted Occupied Bed

Measure hospital staffing intensity as paid FTEs per adjusted occupied bed, compare it with a benchmark, and translate the gap into FTEs and annual labor cost.

Payer Mix & Contractual Allowance Estimator

Estimate net patient revenue and contractual allowances from gross charges and expected collection rates by payer, and compare payer mix on a gross and a net basis.

Operating & EBIDA Margin Calculator

Measure hospital profitability the way rating agencies and lenders do: operating margin, operating EBIDA margin and excess (total) margin including non-operating income.

Average Daily Census & Occupancy Calculator

Measure hospital bed utilization: average daily census, occupancy of staffed and licensed beds, average length of stay and bed turnover, plus the beds needed to reach a target occupancy.

Covenant Compliance Tracker

Test hospital and health-system financial covenants in one place: debt service coverage, days cash on hand, current ratio and debt-to-capitalization, with the cushion or shortfall for each.

Choosing the right calculator

  • Liquidity and debt: monitor Days Cash on Hand and test all major covenants together with the Covenant Compliance Tracker.
  • Revenue cycle: track Days in A/R & Net Collection Rate and estimate net revenue with the Payer Mix & Contractual Allowance Estimator.
  • Volume, acuity and cost: measure Average Daily Census & Occupancy, Case Mix Index and Cost per Adjusted Patient Day.
  • Productivity and profitability: benchmark staffing with FTEs per Adjusted Occupied Bed and track Operating & EBIDA Margin.

Frequently asked questions

What are the most important hospital financial metrics?

Operating margin, operating EBIDA margin, days cash on hand, debt service coverage, days in accounts receivable, and cost and staffing per adjusted patient day are among the most widely monitored by boards, lenders and rating agencies.

Why do hospitals adjust patient days for outpatient activity?

Outpatient services make up a large share of hospital activity. Adjusted patient days convert outpatient revenue into inpatient-day equivalents, so cost and staffing can be compared across hospitals with different outpatient mixes.

Do covenant calculations follow a standard definition?

Not entirely. Bond indentures, bank loans and HUD-insured mortgages each define covenants such as debt service coverage and days cash on hand. The calculators use common definitions and note where your documents may differ.
